EOPS Progress Report

It’s progress report time! Remember, you must complete and submit a progress report to be eligible for the EOPS textbook service, supply service and grant.  The Spring 2024 progress report deadline is Sunday, April 14 at 11:59 PM.

To be considered for the $450 grant, textbook and supply services for Fall 2024, you must complete the progress report by the deadline.  To be eligible, students must follow the EOPS Mutual Responsibility Agreement, which includes submitting a progress report. 

To submit your progress report, complete the online progress report form. If you contact your instructor and do not receive a response, we recommend you make a second attempt to contact them. If you still do not hear back, submit your progress report anyway by simply copying and pasting the request for information that you sent to your instructor in the comments. Again, if you are missing instructor feedback, submit your progress report by the deadline.  

You only need to complete the online progress report once, even if you participate in another special program.   

Submit your online progress report form, by completing these steps: 

2. Late start class. Indicate whether or not your class is a late start class.

3. Current grade.  Look it up on Canvas or contact your instructor as part of step two.  If you don’t have a grade yet, please indicate N/A. 

4. Feedback from your instructor(s).  Contact your instructors through Canvas or e-mail to obtain their feedback.  Then, copy/paste their response(s) into the online progress report.  If your instructor(s) does not respond, copy/paste the request for information that you sent to your instructor in the comments.

Here is a sample message to your instructor: 

Dear Professor, 

My name is (write your name) and my student ID is (write your student ID). I am currently enrolled in the following class (write the class you are enrolled in). As an EOPS student, I would like to know my current grade (if available) and feedback on how I am doing in your class.  Any comments or recommendations you may have on how I can improve my grade in your class would be helpful.   

Please respond by (insert date)

Should any of your instructors have a prolonged absence or if you have any problems completing your progress report, contact the EOPS Team so that we can help you find an alternative solution. Contact the EOPS Team by visiting the EOPS Office during regular business hours.
