Covid-19 Symptom Self-Check Procedure
To keep our students and employees safe, the LACCD is following the Los Angeles County Public Health Department recommendations and requiring all students and employees to be assessed for COVID-19 symptoms each day before entering any LACCD campus or facility.
Student & Employees Health Screening Questionnaire
Students and employees need to complete the daily health screening through your unique link from Cleared4. Instructions for how to access your Cleared4 pass were been emailed to your school email address from Cleared4.
If you did not receive an email from Cleared4 to set up your account, please contact @email. Remember to wear a mask when indoors.
Visitor Health Screening Questionnaire
Visitors need to complete a health screening questionnaire before entering any college or District facility. Visitors should complete the LACCD Questionnaire for Visitors, which is also available on the LAVC Website or LAVC Safe App.

Reopening Protocol Checklist

The Los Angeles County Department of Public Health (LACDPH) has issued its “Protocols for Institutes for Higher Education” which includes a checklist of items to be completed by colleges and universities as part of the re-opening process. The LACDPH protocols require that the completed checklist(s) be posted on the institute’s website.
Additional signage with a QR code or website link to the website where the completed checklists are posted shall be physically displayed at the key entrance points of the college or university.

LACCD Forms
COVID-19 Leave of Absence Packet
The LACCD Coronavirus Info for Faculty & Staff has information, forms, and instructions for the various types of COVID-19 related leaves that are available to eligible LACCD employees based on representation.
In addition to information and request forms for the employer-paid benefits provided by the Families First Coronavirus Response Act (FFCRA), Paid Sick Leave and Expanded Family and Medical Leave, there is information about leaves for specific COVID-19 related reasons and those who need to extend leave past the leave limits established under the FFCRA.
While employees may need to submit more than one type of leave request form depending upon their individual situation, all forms are contained within the packet which is to be submitted to the location Personnel Office directly. The contact(s) for each location are listed on the last page of the packet.
Any questions related to the leaves, the request forms, or the submittal process should be addressed to the LAVC Payroll & Personnel Office.
