INITIAL CERTIFICATION REQUEST

Please follow the following steps prior submitting your initial certification request:

  1. You must meet with the veterans counselor EVERY semester prior to submitting your certification request.).
    1. If the class includes lecture and laboratory, list the lecture information only.
    2. Courses taken at another institution, must be entered at the bottom of the form under "Parent School Letters Request."
    3. If the class(es) include lecture and laboratory, list the lecture information only.
    4. Do not include classes that are  wait-listed.  Once officially enrolled, you may submit an Enrollment Change email indicating the enrollment changes to the initial certification.  (Please see the "Enrollment Changes" module for additional information.)
    5. It is the student's responsibility to enroll in classes indicated on the Student Educational Plan (SEP).  Therefore, prior submitting a  Certification Request form, you must verify that all courses are on your (SEP).  

After reading the steps above, please complete, upload, and submit the Certification Request below based on semester. 

IMPORTANT:

  • Late start classes not submitted in the initial request, must be submitted as an Enrollment Change. 
  • Summer sessions must be submitted in one certification request.
  • Winter and Summer Full-time unit requirement:
    • 5-week = 4 units
    • 8-wk = 6 units
  • CHAPTERS 31/33 ONLY:
    • To prevent BAH Reduction: Enroll in classes coded as OL LIVE and OL NO LIVE.  You may enroll in INTERNET courses as long as they are combined with an OL LIVE and OL NO LIVE course during the same session (i.e. ANATOMY 1 (OL LIVE) and SPANISH 1 (INTERNET) ).
