REGISTER

REGISTRATION PROCEDURE FOR CAMP

Registration for Summer Camp will open on Tuesday, April 1, 2014 and will remain open until all spots in camp are filled or the Wednesday prior to the start week of camp.  You may pick up forms at the Community Services Office or download them here. PLEASE NOTE: YOU MUST TURN IN COMPLETED FORMS AT THE COMMUNITY SERVICES OFFICE.  

Regular registration hours are from 8:00 am to 4:30 pm, Monday through Thursday at the Community Services Office and on Fridays from 8:00 am to 12:00 noon. The Community Services Office is located in the Field House on Ethel Avenue across from the pool. Click here for a map of campus. PLEASE NOTE THAT WHEN COMING TO OUR OFFICE YOU SHOULD PARK IN PARKING LOT L LOCATED EAST OF ETHEL AVENUE. THERE ARE SPACES RESERVED FOR COMMUNITY SERVICES OFFICE BUSINESS ONLY DIRECTLY OUTSIDE THE OFFICE. IF NONE OF THE RESERVED SPACES ARE AVAILABLE PLEASE PARK IN ANY NON STAFF OR DISABLED SPOT, COME INTO THE OFFICE AND ASK FOR A PARKING PERMIT. DO NOT PARK ON ETHEL AVENUE BETWEEN BURBANK BLVD. AND HATTERAS STREET. Ethel Avenue is reserved parking for faculty and staff only. Parking on Ethel Avenue may result in a parking violation citation.

Credit card registration is available 24 hours/day by fax machine (818) 947-2930. You must complete and submit all camp registration forms before paying by phone or fax. We accept American Express, Discover, MasterCard and Visa credit cards.

Fees include a camp T-shirt (one per summer to wear on camp activity days). Extra camp T- shirts may be purchased for $7.00 at the Community Services Office. ALL forms must be filled out completely before a child may be placed in the camp or assigned to a group.

IMPORTANT NOTE: REGISTRATION IS NOT COMPLETE AND NO SPACE WILL BE RESERVED UNTIL ALL FORMS ARE COMPLETE AND ON FILE AT THE COMMUNITY SERVICES OFFICE. YOU MUST COMPLETE NEW FORMS EACH YEAR.

You may obtain a parent card and a permission to treat/field trip waiver form by calling us at 818.947.2577 x4172 or by downloading and printing them at this site. Once you have completed the forms you may bring them to our office in the Field House or you may send them to us at:

PLEASE NOTE THAT THE COMMUNITY SERVICES DEPARTMENT IS NOT RESPONSIBLE FOR LOST OR MISDIRECTED MAIL AND WE STRONGLY RECOMMEND THAT YOU BRING THE FORMS IN PERSON TO OUR OFFICE.
